| In partnership with Hey Hospitalogists, Specialty practices are caught in a vise: costs climbing, reimbursement shrinking, and 2027 shaping up to squeeze harder on both ends. 84% of medical groups say operating costs are higher than a year ago. Medicare physician payment, adjusted for inflation, has fallen 33% since 2001, and next year's expiring payment boost means practices will see even less. Prosper AI's latest research says there's a $108B answer hiding in plain sight. Not in cutting costs further, but in revenue practices are simply failing to capture. Read the full report for strategies to fix that. Big thanks to Prosper AI for sharing the data early with me. Here's my take. BLAKE'S BREAKDOWN The $108B AI Opportunity for Specialty PracticesFor their latest report, Prosper AI conducted original research — including a phone-access study of approximately 2,000 practices, layered against patient and insurer interaction data accounting for more than $3B in care managed and MGMA benchmarks — into where specialty medical practices are losing or failing to capture revenue. The result: $108B in revenue specialty practices are leaving on the table annually, or roughly $125K per physician, broken into four levers practices already have the ability to pull. ![]() What I found worth digging into:
Prosper argues the real opportunity isn't just patching these four leaks individually, but building toward revenue-guaranteed appointments, where benefits are verified and pricing is communicated before the patient shows up. That matters because it removes the uncertainty about affordability and coverage that drives no-shows, a problem 32% of practices said got worse in 2026, with rates reaching 25% in gastroenterology. Practice leaders will see the bigger point here: a full schedule and a profitable one aren't the same thing. Practices can look booked solid and still be bleeding six figures per physician if calls go unanswered after hours, if a name gets misspelled on an intake form, or if a patient has no idea what they owe until weeks after the visit. With an already squeezed margin and 2027's reimbursement cuts approaching, closing these gaps stops being optional. Check out the full report here, which includes a revenue opportunity calculator to find your specialty’s annual impact per physician. Thanks for the read!
