Here are five moments I keep thinking about:

• 10% of an audit sample becomes 10% of a contract. Dana B on how the 2023 RADV rule turned slow documentation into a contract-wide liability.
• Retrospective review assumes clinicians have infinite memory. Dana B on why asking doctors to recall an encounter months later was never a realistic workflow.
• In risk adjustment, the amputations grow back on January 1. Dana M's fix for gap overload is clinical evidence on every suggestion, provider incentives, and the full year to close them.
